About the Oklahoma Nurse Aide Registry (NAR)

The Oklahoma Nurse Aide Registry (NAR), managed by the Oklahoma State Department of Health (OSDH) Long Term Care Service, is the official database of certified Long Term Care Aides eligible to work in licensed nursing facilities, ICFs/IID, and other long-term care settings across Oklahoma. The registry records certification status, training history, employment eligibility, and any substantiated findings of abuse, neglect, or misappropriation of resident property.

Unlike most states, Oklahoma accepts results from three different OSDH-approved testing vendors - D&SDT-Headmaster (the most widely used), Prometric, and the Health Certification Project (HCP). Once a vendor reports a passing score on both the knowledge and skills evaluation, OSDH adds the candidate to the NAR automatically and posts their status publicly at phin.state.ok.us/narswbsearch.

OSDH staff handle reciprocity applications, renewal questions, and disciplinary inquiries Monday through Friday between 8:00 a.m. and 5:00 p.m. Central Time at (405) 426-8150 or [email protected]. Renewal applications and reciprocity packets are mailed to the Nurse Aide Registry at PO Box 268816, Oklahoma City, OK 73126-8816.

Frequently Asked Questions

How do I verify an Oklahoma CNA license?

Go to phin.state.ok.us/narswbsearch (the OSDH Nurse Aide Registry Search). Enter the aide's first and last name, or registry number. The free public search returns current status, certification number, expiration date, and any substantiated abuse/neglect or misappropriation findings. It is available 24/7.

How often must Oklahoma CNAs renew?

Every 24 months. To renew, you must document at least 8 hours of compensated nursing or health-related services during the previous 24-month certification period. Renewal applications are submitted to the OSDH Nurse Aide Registry. There is no continuing-education hours requirement for renewal, although employers are required to provide annual in-service training.

Which testing vendor's results does Oklahoma accept?

Oklahoma is unusual in accepting results from three OSDH-approved vendors: D&SDT-Headmaster (the most common, via hdmaster.com), Prometric, and the Health Certification Project (HCP). All three test the same NNAAP-aligned knowledge and skills content and submit passing results directly to the OSDH Nurse Aide Registry for automatic registry placement.

Does Oklahoma accept out-of-state CNA reciprocity?

Yes. CNAs in good standing on another state's nurse aide registry can apply for reciprocity by submitting the NAR-RECIP application along with proof of current certification and at least 8 hours of paid nursing-related work in the past 24 months. OSDH verifies your status directly with your home state's registry and adds you to the Oklahoma NAR without requiring you to retake the competency exam.

What can result in being removed from the Oklahoma Nurse Aide Registry?

Under OAC 310:677, OSDH can post substantiated findings of resident abuse, neglect, or misappropriation of property on a CNA's NAR record. Such findings remain on the registry permanently per federal OBRA rules, and the CNA may not legally work in any licensed long-term care facility. Certification lapse for failure to renew or to work the required 8 hours also makes the CNA ineligible until they reinstate or retest.

How do I contact the Oklahoma Nurse Aide Registry?

Call (405) 426-8150 or email [email protected]. Mail correspondence goes to Oklahoma State Department of Health, Nurse Aide Registry, PO Box 268816, Oklahoma City, OK 73126-8816. Office hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. Central Time. The fax number is (405) 900-7572.
